Resource Type: This reporting form is used by Eleazar Partnerships in California to obtain information from churches/FBOs that seek to join their network. Despite this specific purpose, it can serve as a helpful self-assessment tool for churches/FBOs to examine their own cooperation with others; their degree of penetration in a neighborhood (relationships with neighbors, facilitating neighborhood meetings, identifying/recruiting neighborhood leaders); the extent of their familiarity with Asset-Based Community Development; and the progress they have made so far in terms of encouraging greater neighborhood solidarity, fellowship, and shared community action.

Main Audience: Churches and Christian non-profits that want to self-assess their degree of progress in truly working “with” a neighborhood (instead of just doing ministry “in” or “to” a community).
